Face palm moment, but lesson learned - no 2 list items should ever have the same value if you want to do a postback when the selected index changes.

Make sure you don't populate the Gridview in the Page_Load as it will fire before the Selected Index Changed.
If you do, the row on the Gridview, your dropdown is on will be reloaded and hence won't technically be there anymore by the time it needs to fire the Selected Index Changed method.
